A free physician review comes first. You pay nothing until Dr. Akram accepts your enrollment into the Ayah Health Wellness Program and you've agreed to all terms.

Our intake chat asks the questions. Dr. Akram curates each plan, personalized to you based on your answers and needs. The chat never orders, prescribes, or decides on a medicine.

    Dr. Akram reads your intake and decides whether to accept you, ask for more information, or explain why another kind of care would serve you better. This review costs nothing.

    After you're accepted, you'll see the full terms and agree to recurring billing in its own step. Your first month is $150 and includes your initial wellness assessment and plan. After that, membership is $99 per month.

Insulin resistance, prediabetes, weight and body composition, fatty liver risk, and the quality and timing of what you eat.

A Good Fit If

You have weight goals, prediabetes or early type 2 diabetes, fatty liver, a larger waist measurement, or PCOS with metabolic features.

Reviewed first: Weight-loss targets, calorie advice, and fasting wait for Dr. Akram's clearance if you have a history of disordered eating, are pregnant or breastfeeding, have type 1 diabetes, or take insulin or a sulfonylurea. Questions about weight-loss medicine go to Dr. Akram, and the chat never recommends one.

Heart Health and Longevity

Blood pressure, cholesterol, cardiovascular risk, fitness, and healthy aging, including bone, brain, and cancer screening.

A Good Fit If

You're 40 or older or have a strong family history, you have high blood pressure or cholesterol, or your goal is healthy aging.

Reviewed first: Chest tightness, unusual shortness of breath, or leg cramping when you walk that eases with rest go to Dr. Akram before any exercise plan. High-intensity work waits for clearance after a recent cardiac event or while your blood pressure is uncontrolled.

Stress, Mood, and Resilience

Chronic stress, burnout, mild-to-moderate anxiety or low mood, coping skills, connection, and a sense of meaning.

A Good Fit If

You carry high stress, screened positive on a short mood and anxiety check without a severe result, are a caregiver under strain, or lean on alcohol or food to cope.

Reviewed first: This track builds stress-management skills and habits. It isn't treatment for depression or anxiety disorders. A high screening score, or any thought of self-harm, brings Dr. Akram in before any track content, and 988 information appears right away.

Sleep, Energy, and Recovery

Insomnia patterns, your body clock, sleep apnea screening, fatigue, and strategies for shift work.

A Good Fit If

You sleep too little or too poorly, feel tired all the time, work shifts, snore, or have a new baby at home.

Reviewed first: Signs of possible sleep apnea, such as loud snoring with daytime tiredness, prompt a conversation about a sleep study before any advice to restrict your sleep. Drowsy driving gets an immediate safety message. Fatigue with red flags, such as unintended weight loss or night sweats, goes to Dr. Akram for a medical workup first.

Digestive and Gut Health

Bloating, constipation, diarrhea, reflux, and other functional symptoms, along with the food triggers behind them.

A Good Fit If

You have ongoing digestive symptoms with no warning signs, IBS-type patterns, or reflux.

Reviewed first: This track only opens when there are no warning signs. Blood in the stool or black stool, unintended weight loss, trouble swallowing, persistent vomiting, or symptoms that wake you at night send you to Dr. Akram for evaluation first.

Women's Health and Hormonal Balance

Cycle health, PCOS, PMS and PMDD, perimenopause and menopause, plus preconception and postpartum health.

A Good Fit If

You have cycle, hormone, or menopause concerns.

What We Follow

  • How many hot flashes do you have on a typical day, and how many night sweats a week?
  • How bad was your period pain this month, from 0 to 10?

Reviewed first: Very heavy bleeding, any bleeding after menopause, a breast lump or nipple discharge, and severe pelvic pain go to Dr. Akram first. Fasting and herbal advice are held back during pregnancy, and herbs meant to bring on a period or raise milk supply are never suggested without approval.

Men's Health and Vitality

Energy, drive, body composition, urinary symptoms, testosterone questions, fitness, and prostate screening.

A Good Fit If

You've noticed changes in energy, drive, sexual function, or urinary symptoms, or you feel a midlife decline.

Reviewed first: A testicular lump or pain, blood in the urine, painful or blocked urination, or erection problems together with chest pain go to Dr. Akram first. Any steroid, SARM, or non-prescribed testosterone use goes to Dr. Akram before advice, and the program doesn't recommend "testosterone booster" supplements. Ayah Health doesn't prescribe testosterone.

Strength, Movement, and Pain

Strength training, mobility, joint and back pain, injury recovery, and functional fitness.

A Good Fit If

You're mostly sedentary, live with joint or back pain, are recovering from an injury, worry about losing muscle as you age, or have sport and performance goals.

Reviewed first: Pain that wakes you at night or happens at rest, numbness or weakness, bladder or bowel changes, fever or weight loss with pain, a major recent injury, or a hot, swollen joint go to Dr. Akram before any exercise plan.

Whole-Person Foundations

A structured 12-week on-ramp across all six pillars, plus getting your screenings and vaccines done.

A Good Fit If

You're generally healthy, have several small goals, are new to lifestyle change, or no other track clearly fits. At your quarterly review, Dr. Akram can move you to a specialized track if a specific issue has emerged.

Reviewed first: This track has no extra holds of its own. The safety checks that run for every patient apply here too.

Sample questions are illustrative. The chat asks them conversationally, using your own goals and last month's action items.

What the Intake Asks

Up to 40 questions, in an order meant to feel comfortable: motivating questions first, sensitive topics only after some rapport. A typical patient answers about 34 to 37, because questions that don't apply to you drop out.

Built on the six pillars of the American College of Lifestyle Medicine, the American Heart Association's Life's Essential 8, and the screenings recommended by the U.S. Preventive Services Task Force.

Your Monthly Check-In

Every month, everyone answers the same core questions on all six pillars, whatever their track. Your track's questions come next, then a short close: anything you want Dr. Akram to look at, and what to focus on next.

It starts from your own goal and last month's action items rather than generic questions. Areas that are stable and going well get one quick question, and deeper ones appear when something slips. It's designed to take about 15 to 20 minutes.

Every third check-in adds a quarterly review of your wellbeing, goals, and track, with video if you prefer. Every sixth adds a six-month refresh of diagnoses, medicines, screenings, and vaccines. Your annual wellness review includes clinically appropriate lab ordering and results review; lab testing is paid separately.

Six Pillars, Every Month

  • Nutrition

    Eating quality, whole-food days, vegetables and fruit, sugary drinks, and eating late at night.

  • Movement

    Days of moderate activity, strength days, steps if you track them, and any pain limiting you.

  • Sleep and Energy

    Hours, quality, nights with trouble, snoring, and how your energy holds up.

  • Stress and Mood

    Your stress score, minutes of calming practice, and the same four-question mood and anxiety screen as intake.

  • Connection

    How connected you feel to people, and any big life events that changed your month.

  • Substances

    Alcohol in a typical week, and nicotine and cannabis compared with last month.

Every check-in also asks about new symptoms and any change to your medicines or supplements. If you have a blood pressure cuff, smart scale, or wearable, you can add those readings, and you can upload new lab results.

Safety Checks Run Before Advice

Your answers are screened as you give them. What happens next depends on what they suggest.

Possible Emergency

Chest pain or pressure, trouble breathing, fainting, or a sudden severe headache brings up a message to call 911 or go to the nearest ER right away, and flags your chart for Dr. Akram.

Thoughts of self-harm bring up 988 and emergency guidance immediately. This program isn't staffed for crisis response.

Dr. Akram Looks First

Warning signs such as blood in the stool, unintended weight loss, trouble swallowing, heavy bleeding, a breast or testicular lump, or pain at night pause your plan until Dr. Akram has reviewed them. Your track doesn't start until then.

Some Advice Waits

Specific advice is held back until Dr. Akram clears it:

  • Weight-loss targets and fasting, with a history of disordered eating
  • Fasting during pregnancy, or with type 1 diabetes or insulin
  • Herbs and supplements alongside blood thinners or blood pressure or diabetes medicines
  • High-intensity exercise after a cardiac event
  • Cutting back alcohol after heavy drinking

What We Don't Use

Herbal laxatives, "cure-all" remedies, and testosterone-booster supplements. Herbs and supplements are never first-line here, and anything that could interact with a medicine you take needs Dr. Akram's approval first.

Who This Is For

Adults 18 and older who are physically located in Texas or Florida during care. Wellness coaching supports prevention and habits. It isn't emergency care, prenatal or obstetric care, or treatment for depression or anxiety disorders, and Ayah Health doesn't prescribe controlled substances, including testosterone. Screening results are not diagnoses.

Questions People Ask

Who Chooses My Track?

The chat can suggest a track and show its reasoning, but Dr. Akram decides after reviewing your intake. If two tracks fit, your first-choice goal breaks the tie. At each quarterly review you can ask to switch.

Can I Be in More Than One Track?

One at a time. Your monthly check-in still screens all six pillars, so a patient whose top concern is heart health is still asked about sleep, stress, and substances every month.

Do I Have to Answer Every Question?

No. Every question includes "Prefer not to answer," and questions that don't apply to you are skipped. Some answers help Dr. Akram keep your advice safe, so share what you're comfortable sharing.

What If I'm Pregnant, Breastfeeding, or Have Struggled With Eating?

The program adjusts. Fasting, weight-loss, and supplement advice is held back until Dr. Akram has reviewed your chart. Ayah Health doesn't provide prenatal or obstetric care.

How Do I Cancel?

Under Account settings in your patient portal. Cancelling stops future renewals, and your access continues through the period you've paid for. Your records stay yours, and you keep access to plans already released to you.

Is My Information Private?

Your answers become part of your medical record with Ayah Health and are protected under HIPAA. You can read the details in our HIPAA Notice of Privacy.
